Explanation
Micardis (Telmisartan) is an angiotensin II receptor blocker (ARB) indicated primarily for the treatment of hypertension. It selectively blocks angiotensin II at the AT
1 receptor, reducing vasoconstriction and aldosterone secretion.

The usual starting dose of telmisartan for hypertension is 40 mg orally once daily. It may be taken with or without food. The dose may be adjusted within the usual range of 20–80 mg once daily according to blood pressure response.
Sedation and hypoglycemia are not expected effects of telmisartan. Orange juice is not required for administration, and soybean allergy is not a labeled contraindication.
Telmisartan carries a boxed warning for fetal toxicity. Drugs that act on the renin-angiotensin system can cause fetal harm; discontinue telmisartan as soon as pregnancy is detected.
Key Points:
-
Monitor blood pressure, serum potassium, and renal function during therapy.
-
Counsel patients to avoid potassium supplements and potassium-containing salt substitutes unless directed by a healthcare professional.
-
Concurrent use of aliskiren is contraindicated in patients with diabetes.
Exam Pearl: Micardis HCT tablets should not be removed from their blister packaging until immediately before administration.
