Amiodarone 4. Since grapefruit juice is known to inhibit CYP3A4-mediated metabolism
(Tablet) of oral Amiodarone (Cordarone) in the intestinal mucosa, resulting in
(Injection) increased plasma levels of Amiodarone (Cordarone), grapefruit juice
should not be taken during treatment with oral Amiodarone (Cordarone).
5. Because of the food effect on absorption, Amiodarone (Cordarone)
should be administered consistently with regard to meals.
6. Prolonged exposure to Amiodarone (Cordarone) occasionally results
in a blue-gray pigmentation. This is slowly and occasionally incompletely
reversible on discontinuation of drug but is of cosmetic importance only.
7. Nausea, vomiting, peripheral neuropathy, optic neuropathy and/or
optic neuritis, asymptomatic corneal micro deposits and photosensitivity
reactions are reported side effects of Amiodarone (Cordarone).
Dronedarone 400 mg PO bid 1. Dronedarone (Multaq) is indicated to reduce the risk of cardiovascular
(Tablet) with a meal. hospitalization in patients with paroxysmal or persistent atrial fibrillation
(AF) or atrial flutter (AFL).
2. It may cause fetal harm when administered to a pregnant woman. It
is a pregnancy category X drug.
3. It should be administered with a meal. Warn patients not to take this
drug with grapefruit juice.
4. Hypokalemia and hypomagnesemia with potassium-depleting
diuretics, nausea, diarrhea, QT prolongation and heart failure are
reported with Dronedarone (Multaq) therapy.
Sotalol 80 mg to 160 mg 1. Sotalol (Betapace) is indicated for the maintenance of normal sinus
(Tablet) PO bid. rhythm in patients with symptomatic atrial fibrillation/atrial flutter who
are currently in sinus rhythm. It is also indicated for the treatment of
sustained ventricular tachycardia.
2. To minimize the risk of induced arrhythmia, patients initiated or
reinitiated on Sotalol (Betapace) should be placed for a minimum of
three days (on their maintenance dose) in a facility that can provide
cardiac resuscitation and continuous ECG monitoring.
3. Sotalol (Betapace) is not approved for the atrial fibrillation/atrial
flutter (AFIB/AFL) indication and should not be substituted for Betapace
AF because only Betapace AF is distributed with a patient package
insert that is appropriate for patients with AFIB/AFL.
