3. American Society of Health-System Pharmacist accredited residency
4. Other clinical residency
5. Completion of a continuing education certificate program in at least one chronic disease state
6. PharmD degree
7. Master’s degree
8. Specific number of years of clinical experience
9. Board of Pharmacy approved education/training
10. Specified annual continuing education.
